Each Agency Submitting a Funding Application

 

Submit One original and (10) ten copies of each

 

  1. Organization Information Overview
  2. Requested Funding Application(s)- Hole punched and paper clipped (do not staple).

 

Submit only one (1) copy of each Per Agency

 

  1. Board resolution authorizing that person signing Funding Application(s) and Assurances has been duly authorized by the governing body of the applicant if not signed by the board president.
  2. Copy of most recent month’s year-to-date Balance Sheet and Income/Expense Statement for all operations of the organization
  3. Proof of Insurance
  4. Proper signature and date on each Assurance that applies to the Entity’s Funding Applications

 

  1. List of governing board members, address and their affiliation

 

  1. Copy of applicant organization’s full operating budget for the agency’s fiscal year during which the requested funds will be utilized.

 

If this is an off cycle funding request and the above information has already been submitted and has not changed the agency does not have to submit (C, D, E, F)

 

 

Each Agency Submitting a Funding Application for the First Time

Submit

A.    All of the above and the number of copies indicated

B.    Evidence of current non-profit status- submit one (1)

C.    Copy of the most recent IRS 990- submit one (1)

D.   Statement of Corporate Good Standing from the Missouri Secretary of State. May be obtained at no cost from; http://www.sos.state.mo.us/BusinessEntity/default.asp- submit one (1)

E.    The most recent complete audit of your organization- submit one (1)

 

 

 

For all capital projects
A.    Include Bids on all equipment for projects that exceed $500.  Include a bid summary sheet with local, item, date, name of business, bid amount, and which bid is selected.

 

Provider Bidding Requirements:

           The following will be used as a guide in determining when competitive quotations should be obtained, these procedures must be followed.

          

a.     Purchases under $500 - must be approved by the JCSFB funding process

 

b.     Purchases between $500 and $2,500  - Quotations from at least three sources of supply should be obtained, if obtained by telephone, a written record made of the company, the person giving quote, the telephone number and the prices quoted.

 

c.     Purchases between $2,500 and $15,000 – In addition to (b) the purchaser will seek a minimum of three written bids in place ot the telephone bids.  Faxed bids are acceptable.

 

d.     Purchases over $15,000 - A formal advertised competitive bid process will be required.  Quotations will be obtained from at least two suppliers on a sealed bid basis.  The JCSFB Director must be present upon the opening of the sealed bids.  A bid sheet must be generated and presented to the Jasper County Sheltered Facilities Board of Directors listing all specifications and bid prices.

 

e.     Formal Advertised Bid - Notice and publication of an advertisement for five days in a daily newspaper in the county where the work is located, or at least twice over a period of ten days or more in a newspaper in the county where the work is located, and in two daily newspapers in the state which do not have less than fifty thousand daily circulation, and by such other means as are determined to be most likely to reach potential bidders.  (Statute Reference 34.040)

          

          


Organizations are required to complete a business/marketing plan along with the long-term funding plan, including projections of Jasper County Sheltered Facilities Board’s funding share for any new business or expansion project.  This does not include those projects that are requesting funds for replacement of existing equipment or facilities.  All Capital building or renovations projects will be paid utilizing a title company.

 

Capital Expenditure applications for new business or expansion will be considered only if containing the following information

 

1.     Capital Building

a.     Description of Project and benefits to persons served

b.     Projected timeline for initiation and completion

c.     Business Plan as stated above

d.     Land Site and value

e.     Architectural Plans or equivalent

f.      Cost Breakdown for entire project

g.     Bid Letter as sent out to prospective contractors

h.     3 submitted bids including the bid accepted

i.      Estimated Appraised or Market Value upon completion of project

 

2.     Capital Renovation

a.     Description of Project benefits to persons served

b.     Projected timeline for initiation and completion

c.     Architectural Plans or equivalent

d.     Preliminary Cost Breakdown for entire project

e.     Bid Letter as sent out prospective contractors

f.      3 submitted bids including the bid accepted

g.     Estimated Appraised or Market Value upon completion of project
